On Metacritic, the film has a score of 52 out of 100 based on 18 critics, indicating "mixed or average reviews". The film begins with members of the Catholic Church digging up the body of a 19th-century church official, whose casket has a box-shaped urn chained to it.

Detective Enzo Marki ( Cristian Solimeno), the police officer who has been tailing Sarah, joins her to venture into the catacombs where Mater Lachrymarum's cult lurks. She uses this to avoid a police detective, though she is forced to kill a witch, Katerina, who catches and corners her on the train. There are some brilliantly startling jump scares and a “turned up to eleven” version of Dario Argento’s famous gross-out gore effects and cruel ultra-violence that will make even the toughest horror movie fan wince ( and he really does go for maximum shock value here! Tenebrarum is perhaps referring to Suspiria's finale, during which Mater Suspiriorum is killed and her home eventually burns to the ground. The Cannes Film Festival requested that The Third Mother be ready in time for consideration as a 2007 competition contender.

Each film deals with one of the titular "Mothers", a triumvirate of ancient witches whose powerful magic allows them to manipulate world events on a global scale, killing anyone who discover their whereabouts, and who are determined to rule the world. Jim Ridley in The Village Voice felt the film was further evidence of the declining quality of a once great director's abilities, stating that "for people who revere the horror maestro's vital work—roughly the period between his debut, 1970's proto– De Palma giallo The Bird with the Crystal Plumage, and his 1990 segment of the anthology film Two Evil Eyes — it's painful to watch the Hieronymus Bosch of '70s horror sink this low. They talk to the priest, only for him to be killed by one of his patients before he can give the two a copy of a book that would explain Mater Lachrymarum's backstory. In the 2018 remake of Suspiria, the roles of both Madame Blanc and Helena Markos, referred throughout as Mother Markos, are played by Tilda Swinton.

The concluding film of The Three Mothers trilogy provided some reviewers with an opportunity to reflect on Argento's career as a whole, and parallels were often drawn between Mother of Tears and Argento's films from the 1970s and 1980s.During the 2007 Toronto International Film Festival, Argento stated he had not ruled out the possibility of shooting a fourth film dealing with the Three Mothers. On 27 May, a second promotional poster – featuring an eye weeping tears of blood – surfaced on the internet bearing only the title Mother of Tears. In 1895, she founded the Tanz Akademie ("Dance Academy" in German), a school for dance and the occult sciences, in the Black Forest outside Freiburg, Germany.

I had only seen this movie once before on cable tv and I was very disappointed as I am a fan of the first two movies. Since the 70's Argento's very personal and unique view of the thriller / horror genre has divided not only horror movie fans but his own fans to, even the critic and fan Alan Jones has voiced his dissatisfaction with Mother of Tears. At the preview during the Cannes Film Festival, journalist Alan Jones described Simonetti's score as an "unqualified success". It's a brave move and the lowish budget doesn't quite pull it off and this would be my main criticism.Michael visits the Cardinal who sent the urn to the museum, but the Cardinal has had a severe stroke and is now comatose. French horror magazine L'Écran Fantastique reported that Argento alone would receive a screenwriting credit. As excessive and ketchup laden as predecessors Suspiria and Inferno, Dario Argento's Mother of Tears completes the trilogy with the same baroque grandeur and soggy 1970s sensibilities.

However, in early 2006 rumors circulated that Argento had dismissed Anderson and Gierasch after being displeased with their script.Primary filming occurred in Rome, although some parts were filmed in Turin and at the Cinecittà studios in Terni. When Sarah examines an urn discovered at a decrepit grave near Viterbo, she uncovers within it relics of a witch known as the Mother of Tears (Moran Atias). In The Mother of Tears, it is revealed that before the events of Suspiria, Elisa Mandy ( Daria Nicolodi), a white witch, sought to challenge Markos' evil might.
